  By submitting this form, the above-named player is expressing interest in being considered to play on the Jr. Lancers Boys Basketball Program during the 2011-2012 basketball season.
- As parents, we understand that, if accepted to play: We will be responsible for paying our son's registration fee ($385.00) prior to the first day of practice.
- As parents, we understand we are committing to help with concessions and/or other tournament duties during one of two Jr. Lancers Tournaments during the season (Best Of The Best Tournament - November 20, 2011) and (Hoop It Up Classic - January 28-29, 2012). Our organization relies on each family to assist with our fundraising efforts by volunteering their time and efforts during these tournaments.
- Further, we have read and understand the Jr. Lancers Basketball Parent Code Of Conduct which explains the six core principals of character.
- Tryouts will be held at Wisconsin Hills Middle School on Monday, September 26th and Wednesday, September 28th. Attendance at both tryout sessions is mandatory.

Evaluation Schedule

Evaluation Schedule based on school grade player will enter this coming fall
(please sign-in at the gym entrance at Wisconsin Hills):
  4th Grade - 4:00PM to 5:00PM - (9-26 & 9-28 at WHMS)  
  5th Grade - 5:00PM to 6:00PM - (9-26 & 9-28 at WHMS)  
  6th Grade - 6:00PM to 7:00PM - (9-26 & 9-28 at WHMS)  
  7th Grade - 7:00PM to 8:00PM - (9-26 & 9-28 at WHMS)  
  8th Grade - 8:00PM to 9:00PM - (9-26 & 9-28 at WHMS)
